A validated 9-item screen for depression severity used in primary care and mental health settings.
If you've been feeling persistently low, unmotivated, or disconnected, the PHQ-9 can help you put a number on it. This is the same validated 9-item scale doctors and therapists use to screen for depression severity — not a diagnosis, but a clear, evidence-based starting point for a conversation about how you're really doing.
Clinical screening — not a diagnosis. Answer based on the past two weeks unless stated otherwise.
The PHQ-9 measures how often you've experienced nine core depression symptoms over the past two weeks — low mood, loss of interest, sleep and appetite changes, energy, concentration, self-worth, and thoughts of self-harm. It's the most widely used depression screening tool in primary care, and this test uses the exact same items and severity bands (0-4 minimal through 20-27 severe) that clinicians rely on.
Research background. The PHQ-9 is a validated instrument (Kroenke, Spitzer & Williams, 2001) with strong psychometric properties and widespread clinical use. Learn more from the NIMH or APA.
